Service Volunteer Program
Volunteer Contract
Purpose: Welcome Baby was created to provide support and information
to first-time parents. Welcome Baby Partners (volunteers) work
with the parent(s) in an individual relationship.
Partner Responsibilities
- Complete and return the Partner Application
Packet.
- Complete the initial Welcome Baby training.
- Commit to one
(1) year of service.
- Contact mother/family one time per
month.
- Adhere to the signed confidentiality statement.
- Deliver
a "Welcome to Parenting" package to client.
- Complete
Contact Record form for each client contact.
- Submit competed
Client Contact Record form by the 5th of each month.
- Assist
client in identifying meaningful goals.
- Assist client in
accessing community resources.
- Report any change of contact
information to Welcome Baby staff.
- Complete and return
Volunteer Satisfaction Survey for each client.

TO WELCOME BABY PARTNERS FOR PURPOSES OF TRANSPORTING FAMILIES.
Parent Contract
Purpose: Welcome
Baby was created to provide support and information

Parent Responsibilities
- Will assist Welcome Baby staff to complete
home visit/intake
- Will complete and return Partner Preference/Assessment
form
- Will commit to three (3) months to one (1) year of
service
- Will have at least one (1) contact per month with
their partner
- Will inform Welcome Baby staff of any concerns
- Will communicate
to partner self identified goals
- Will utilize partner information
to achieve self identified goals
- Will access appropriate
community resources
